New patients at Crown Point Smiles can expect a first visit focused on understanding their dental health, concerns, and goals. We explain our findings clearly and help you make informed decisions about your care.
Schedule Your First Visit
You can request an appointment online or call Crown Point Smiles at (219) 226-3189. When scheduling, tell us about urgent dental concerns, insurance questions, or accommodations that may help you feel comfortable.
Complete Your New Patient Forms
Save time at check-in by downloading and completing your new patient forms before your appointment. Bring the completed forms with you, and contact our team if you have questions about any requested information.
What to Expect at Your First Appointment
Your first appointment gives our team a chance to understand your oral health and the reason for your visit. Depending on your needs, it may include:
- A review of your medical and dental history
- A conversation about your symptoms, concerns, and goals
- An examination of your teeth, gums, bite, and surrounding tissues
- Dental X-rays or other diagnostic images when appropriate
- An assessment of your gum health
- A discussion of our findings and recommended next steps
If treatment is recommended, your dentist will explain the available options and answer your questions before care moves forward.
Will I Receive a Cleaning at My First Visit?
A professional cleaning may be completed when it is appropriate for your oral health and appointment type. Some patients need an examination or additional assessment before the right type of cleaning can be determined, and treatment may be scheduled for another visit.
What to Bring
- A valid photo ID
- Your current dental insurance card
- A list of medications you currently take
- Relevant medical information that may affect your dental care
- Recent dental records or X-rays, if available
Transferring Dental Records and X-Rays
If another dental office has recent records or X-rays, contact that office before your appointment and ask how to have them transferred. Contact Crown Point Smiles if you need help coordinating the transfer.
Dental Insurance and Payment
Visit our Dental Insurance page for guidance about confirming benefits. Network participation, deductibles, limitations, and benefits vary by plan. Our team can review information supplied by your insurer, but your insurance company determines final coverage and payment.
