Dental care can be easier to plan for when you know what to expect. The Choice One Savings Plan gives patients without traditional dental insurance another way to manage the cost of preventive care and receive savings on eligible treatment at participating offices.

This is a dental savings membership, not dental insurance. Members pay an annual fee and receive the services and discounts described by the plan while their membership remains active.

  • One emergency exam

How the Dental Savings Plan Works

  1. Choose a membership. Review the available adult or child option and its current benefits.
  2. Enroll and pay. You can subscribe online through the secure Choice One portal or ask our office for assistance.
  3. Schedule your dental care. Contact Crown Point Smiles when you are ready to arrange an appointment.
  4. Receive applicable benefits. Included services and member discounts apply according to the current plan terms.

Choice One states that active members can begin using the plan without a waiting period and that the plan does not place a cap on eligible savings. Members may also add dependents, with the adult dependent rate shown separately from the primary-member rate.

Is a Savings Plan the Same as Dental Insurance?

No. Choice One is a licensed discount savings plan offered through participating providers. It does not pay claims to Crown Point Smiles or reimburse members for dental treatment. Members remain responsible for paying the cost of care after any applicable plan discount.

The plan is not a qualified health plan under the Affordable Care Act. It should not be described or used as dental insurance. If you already have dental coverage, contact our team before enrolling so you can understand how the available payment options may apply to your care.

Who May Want to Consider the Plan?

The Choice One Savings Plan may be useful for individuals or families who do not have traditional dental insurance and expect to receive routine preventive care. Your individual needs, anticipated treatment, and total costs determine whether the plan offers value for you.

Before enrolling, review the current Savings Summary, participating-provider information, limitations, renewal terms, cancellation terms, and fees on the official website.
